What's happening to our lawn?

Problem - Our lawn was a patchy mess of weeds and crap so we had our lawn resodded with shady fescue. It looked great. We started our maintenance routine so it would stay great:

1. made sure it got a good soaking at least once a week
2. Four treatments a year. Spring - Crabgrass preventer with lawn food. Later Spring - Lawn food. Early Fall - Crabgrass preventer with lawn food. Late Fall - Lawn food.
3. Cut it when needed (not too short)
4. Spot treat for weeds with non-grass harming crabgrass spray

We also worked with the local extension office before we resodded and added the right about of chemicals to the soil. It stayed great for about 8 months. Then it started dying in patches. We don’t know why.

Location - Southeastern US. Humid climate.

Soil Conditions - Soil and clay. Have not seen water pool so I assume it drains well

Which side of your house - Southeast. Gets maybe 4-5 hours of sunlight?

1) You may have grubs - NO GRUBS WERE FOUND UPON DIGGING

2) The sod may not have attached to the soil - I PULLED ON SECTIONS AND THEY SEEM TO HAVE ROOTED FINE
